The game evaluates three distinct variables every time Wintertodt unleashes a cold attack: your Firemaking level, your base Hitpoints level, and the quantity of warm items worn. Players often misinterpret the mechanic as a flat defensive soak. Instead, cold attacks hit for a calculated percentage of your maximum Hitpoints, which makes high-Hitpoints accounts take significantly harder hits than fresh level-3 accounts.
Wearing warm items scales your protection up to a hard ceiling of 4 equipped pieces. Each worn piece reduces the base damage formula:

Going from zero warm items to 4 warm items cuts the initial multiplier in half.
