The 423 area code map holds a rare distinction within American telecommunications: it is divided into two disconnected geographic pieces. When the region split from Nashville's 615 area code in 1995, 423 initially stretched across nearly the entire eastern half of the state.

That arrangement lasted only four years. In 1999, regulators partitioned the Knoxville metropolitan area into the 865 code. This carve-out sliced the 423 footprint into two non-contiguous sections. The southern section covers Chattanooga, Marion County, and Bradley County, while the northern section covers the Tri-Cities region, including Johnson City, Kingsport, and Bristol, running down through Greeneville and Morristown.
