Eyeglass lenses are not uniform sheets of corrective glass. Every prescription lens features an optical center: the exact point where light passes through without refracting or bending off-target. For optimal focal point accuracy, this optical center must sit directly in front of your pupil when your eyes rest in a natural forward gaze.
Pupillary distance, measured in millimeters, tells optical laboratory technicians where to grind that central focal point into your frames. If your lenses are cut without an accurate PD, your eyes are forced to look through the perimeter of the prescription rather than its sweet spot. Modern high-index and progressive lenses have very tight margins for error; being off by as little as 2 mm can compromise clarity and cause significant visual fatigue.
While lens cutters historically set optical centers to standard frame defaults, digital lab surfacing requires millimeter-level precision. This is particularly crucial for stronger prescriptions, where the curvature of the lens steepens drastically away from the center.
